Diabetes is a condition marked by high levels of glucose (sugar) in the blood. Since 2003, the prevalence of diabetes in the United States has grown by 14%, to nearly 21 million people. Approximately 80% of the diabetics have type 2 diabetes, 10% have type 1, and 10% have another form. People can acquire diabetes at any age.

The Concord Hospital Diabetes Self-Management Education Program gives people with diabetes a chance to learn more about living with the disease. The program offers individual and group classes, and teaches patients how to use insulin pumps and continuous glucose-monitoring sensors.

Patients are trained by registered nurses and registered dietitians who are Certified Diabetes Educators. The American Diabetes Association recognizes this education service as meeting the National Standards for Diabetes Self-Management Education. It is covered by most insurance companies, including Medicare.
