- Review the information in your medical record.
- Request a copy of your medical record. This often involves a fee.
- Request that your medical record be released to someone else.

How to request a medical record:
Patient Online is a free service that lets you manage your healthcare online. You can view parts of your medical records through Patient Online.
Details:
- It usually takes seven to ten working days to release a medical record. Please let us know if you have an urgent deadline.
- To protect the confidentiality of our patients, we can only fax medical records in extreme emergencies.
- At a patient's approval, an insurance company will usually send a request for release of medical records directly to us. We charge a fee of $15.00 to fulfill this request.
- To complete a disability form, fill out and sign your portion of the form, then mail or return it to your family practice site or Dartmouth-Hitchcock Nashua address. Mailing addresses. It usually takes 5 to 7 working days to complete a disability form once we receive it.
