Appointments and Referrals
Depending on the treatment you require for your condition, you may need to meet with more than one physician in the Cerebrovascular Disease and Stroke Program.
Your primary care physician (PCP) can make a referral to the appropriate section by contacting the DHMC Physician Connection Center at (866) DHMC-DOC or (866) 346-2362.
Please refer to your appointment letter for details on where you need to go for your appointment. In general, you will report to Level 3C (PDF) for neurology and neurosurgery-type appointments, and report to Level 3L (PDF) for radiology-type appointments.
If you are a current patient, you may use myD-H to communicate securely with your providers, request or change appointments, review your medical record and more.
For referring physicians
Referring providers can refer patients to the Cerebrovascular Disease and Stroke Program through the DHMC Referring Physician Services program.
Call the DHMC Physician Connection Center at (866) DHMC DOC or (866) 346-2362 toll-free, or locally at (603) 653-1999 to schedule appointments, consult with a physician, or obtain status of patient care. The office is open Monday through Friday from 8:00 am to 5:00 pm.

Referring patent foramen ovale (PFO) patients
A strong collaboration between Interventional Cardiology and Neurology at DHMC has allowed us to be involved in PFO clinical trials for several years.
Patients referred for possible PFO closure are evaluated by both services, assuring that patients undergo a comprehensive evaluation and the most appropriate treatment decisions can be made.
