Home Based Self-management and Cognitive Training Changes Lives (HOBSCOTCH)


People with seizures have a lot of difficulties with day-to-day memory, which may affect their job and their social lives.

HOBSCOTCH (HOme-Based Self-management and COgnitive Training CHanges lives) is a program developed at Dartmouth College specifically for people with epilepsy, which may improve memory performance in people who suffer from seizures.

Memory problems in epilepsy

Every person experiences memory lapses at some point in their life. We have all had moments where we have forgotten someone's name or telephone number, forgotten to take our medication, or forgotten something that was told to us in conversation. Some memory problems can be part of the normal experience of being a human. However, approximately half of persons with seizures report more than average memory difficulties. Persons with seizures or epilepsy have many additional reasons to have memory and concentration problems.

Components of memory impairment in epilepsy include:

  • Abnormal brain chemistry
  • Abnormal brain activity (seizures, brief blips)
  • Medications
  • Stress, anxiety, or poor mood
  • Structural brain abnormalities

HOBSCOTCH (HOme Based Self-management and COgnitive Training CHanges lives) is a behavioral program designed to address memory and attention problems for people who have epilepsy. It uses certain psychological strategies (self-management and problem-solving therapy) to improve memory function in patients with seizures.

The program is designed to help adults with seizures to find ways to manage and cope with memory problems in order to lead a happier, more productive life. The program is simple, cost effective and requires only a few visits to your doctor’s office.

Program structure

HOBSCOTCH consists of two 2 in person at the beginning and the end of the program and 6 telephone sessions, with each session lasting 45 to 60 minutes. The program is designed to be completed over 3 months.

In each personal and telephone visit, participants will learn memory skills and strategies, then practice these skills and apply problem-solving techniques in everyday, real-life situations between visits.

The components of HOBSCOTCH include:

  • Education to teach basics of memory function, normal memory problems, and factors related to seizure-related cognitive problems
  • Self-awareness training, emphasizing awareness of the multiple influences that affect memory and attention
  • Compensatory strategies or skills training to compensate for cognitive functions that may affected
  • Problem solving therapy to provide the participant with skills to systematically work through memory related difficulties and other problems faced
