Appointments and Referrals

As a Family Medicine patient, you will select one of our providers to be your primary care provider (PCP). Your PCP will coordinate your care with other departments.

New patients

At your first meeting with a family medicine provider, we will perform some testing and evaluate your current condition. We'll discuss your medical history, medications and plan of care.

Returning patients

If you are a returning patient, you may use myD-H to communicate securely with your providers, request or change appointments, review your medical record and more.
