New patients
Appointments with the Headache Center are primarily made through our Referring Physician Services program. Please ask your doctor to refer you to our center.
Note: You are required to have a primary care provider (PCP) before you can make an appointment.
Preparing for your appointment
Please print the following form, complete it, and bring it to your appointment. You should plan to fill this form out each time you come in for a visit. This form gives the provider important information to focus in on how best to help you.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.
