Jack Byrne Center for Palliative & Hospice Care Team
Only providers who are members of the Dartmouth-Hitchcock Palliative Care team admit and direct the care of patients at the Jack Byrne Center. If you wish, your own provider can remain connected to your care by visiting you at the Center and by speaking with your D-H provider.
If you are in hospice care, your hospice team will continue to coordinate and guide your plan of care while you are at the Jack Byrne Center, just as they do when you are at home. Your hospice team may visit you while you are here. The Jack Byrne Center team will work closely together with your hospice team.
Many people make up your interdisciplinary team. Each team member has a different role in your care, but all are experts in their own fields.
