Appointments and Referrals
In most cases, you are referred to the Dartmouth-Hitchcock Transplant Center by your nephrologist (kidney specialist), endocrinologist, or another doctor.
Once you have been referred to the Dartmouth-Hitchcock Transplant Center, your first step will be to attend our orientation and evaluation session.
If you are a current patient, you may use myD-H to communicate securely with your providers, request or change appointments, review your medical record and more.
For referring physicians
To refer a patient for consultation in the Transplant Center, contact the DHMC Physician Connection Line at 1-866-DHMC-DOC (1-866-346-2362). You also may contact us at one of our locations directly.
Location and contact information
The Transplant Center is located at Reception 2M on Level 2. Initial appointments are seen at Reception area 4L on Level 4.
Orientation and evaluation sessions are held in the second floor conference room. Individual exams with you take place on the third floor.
