New patients
In most cases, you are referred to Dartmouth-Hitchcock (D-H) Transplantation Surgery by your nephrologist, endocrinologist, or another doctor. Once you have been referred, your first step will be to attend a new patient education class and evaluation session.
Living Donor consent form
If you are interested in becoming a living kidney donor, we require that you sign the form below, "Consent to be Evaluated as a Living Donor". Mailing the printed and signed form to our office will help expedite the donor evaluation process. The consent form is also in the Donor Information Packet, so you can also wait until you receive the packet before signing the form. For referring physicians
To refer a patient for consultation in Transplantation Surgery, contact the DHMC Physician Connection Center. You also may contact us directly at one of our office locations.
Referrals must be accompanied by the Referral form for new patients (PDF) and the Incoming Authorization Form for the patient's medical records.
