New patients
If you are new to the Neurology Memory Program and would like to schedule a consultation or get more information, call us between the hours of 8:00 am and 5:00 pm.
You must be referred to the Cognitive and Behavioral Neurology Program by your primary care provider or other physician, and will not be able to set up an appointment without a physician referral.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.
