Total Pancreatectomy with islet auto transplantation (TPIAT) referral
If you are a provider who would like refer your patient, please fill out the TPIAT referral form and fax to the number on the form. If you are a patient suffering from chronic pancreatitis wanting to be evaluated, please have the referral form completed and faxed to us, or call our office for assistance. Please include all the information requested with the referral form.
