New patients
You must be referred to the Diabetes Program by a physician. Patients cannot set up an appointment without a referral. Please contact your primary care provider (PCP) to request a referral.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.

Our hours are Monday through Friday, 8:00 am to 5:00 pm, unless noted otherwise below:
- Dartmouth Hitchcock Clinics Nashua: Monday through Friday, 7:30 am to 2:00 pm
- Dartmouth Hitchcock Clinics Manchester: Available by appointment
