New patients
You must be referred to the Department of Endocrinology by a physician. Patients cannot set up an appointment without a referral.
Some health insurance policies require that you have a referral from your family doctor (primary care provider, or PCP) to make an appointment. Please review your health insurance coverage before calling for an appointment.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.

Please send all relevant information (notes, labs, and imaging) when making a referral. Additional information is required for specified conditions. Please refer to the Endocrinology Referral Guidelines (PDF) for more information.

Our hours are Monday through Friday, 8:00 am to 5:00 pm, unless noted otherwise below:
- Dartmouth Hitchcock Clinics Concord: Monday through Thursday, 8:00 am to 5:00 pm; closed Fridays
- Dartmouth Hitchcock Clinics Nashua: Mondays, 8:00 am to 2:00 pm
