As a Family Medicine patient, you will select one of our providers to be your primary care provider (PCP). Your PCP will coordinate your care with other departments.
New patients
At your first meeting with a family medicine provider, we will perform some testing and evaluate your current condition. We'll discuss your medical history, medications, and plan of care.
We will also:
- Provide you with information about the New Hampshire Immunization Information System (NHIIS) vaccine registry
- Discuss your participation in the registry
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.

To refer a patient for consultation, contact the Physician Connection Center. You may also contact us at one of our locations directly.
Office hours
Standard office hours are typically Monday through Friday, 8 am to 5 pm.

Dartmouth Hitchcock Clinics Milford Temporary Relocation beginning Monday, April 27
Due to building upgrades taking place this spring, we will be temporarily relocating our Family Medicine team to our Nashua and Merrimack sites beginning Monday, April 27 (for approximately 3 months).
