Dartmouth-Hitchcock Clinic is among 10 physician-groups selected by the Centers for Medicare and Medicaid Services (CMS) to participate in the on-going pay for performance initiative for physicians.
The two-year demonstration project, which began January 1, 2011, will study whether improving care in a proactive and coordinated manner also reduces costs.
Dartmouth-Hitchcock will apply care management practices and programs used with commercially managed care enrollees to Medicare fee-for-service beneficiaries. Disease management initiatives will focus on patient outreach and education surrounding preventive care services and follow-up for targeted chronic disease programs. Disease management and case management programs will focus on congestive heart failure, diabetes, coronary artery disease, hypertension, advanced pulmonary disease, and on beneficiaries with high-cost or complex medical conditions.
