New patients
Thank you for choosing our care team for your medical care. If you are a new patient, please ask your primary care provider (PCP) or other physician to forward a referral with all pertinent medical records. If you would like to be seen at the Multiple Sclerosis Center for the first time, you will need to have a referral by your current doctor. Please ask your doctor to contact us.
Once we receive your referral, we will contact you to schedule your appointment. We may recommend testing before your appointment, which we can help you coordinate. The scheduling secretary will explain your appointments to you in detail.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.
