If you are a new patient, please ask your primary care provider (PCP) or other physician to forward a referral with all pertinent medical records. Patients cannot set up an appointment without a referral. Please see our Referrals and Precertifications page for more information about referrals.
Once we receive your referral, we will contact you to schedule your appointment. We may recommend testing before your appointment, which we can help you coordinate. The scheduling secretary will explain your appointments to you in detail.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.

Office hours
Office hours are 8:00 am to 5:00 pm, unless otherwise noted below.
- Notre Dame Pavilion, Catholic Medical Center: Patient appointments are Thursdays from 8:30 am to 2:00 pm
- Dartmouth Hitchcock Clinics Nashua: Patient appointments are Wednesday, Thursday, and Friday from 8:30 am to 2:00 pm
