Badge Access Orientation

The Patient Safety Training Center (PSTC) offers 24-hour backdoor badge access to select staff. In order to apply for this access, please watch the following video and then complete the forms below it.

To receive PSTC backdoor badge access, you must complete the following:

1. PSTC Badge Access Orientation Video Attestation form
2. PSTC at Dartmouth Hitchcock Medical Center Ground Rules for Participation form
3. SBER Media Consent and Release Authorization form
4. Once these forms are complete, please email colin.a.treem@hitchcock.org to request PSTC badge access.
