Referring a patient to the Department of Psychiatry
Referring an adult
If you're a physician and would like to refer an adult patient, we will need:
- A diagnosis
- The reason for the referral
- The requested service (medication management, therapy, neuro-psychiatry testing, or other specified service)
It is helpful to have any office notes and past testing if available. Please complete the Outpatient Referral Form (PDF). Then fax the form with any additional information to the number at the top of the form.
