APRN Boot Camp

This comprehensive review for certification in geriatrics is aimed at Advanced Practice Nurses currently certified in adult, family, and acute care.

Certified Geriatric Advanced Practice Nurses and Geriatricians will deliver the core identified in American Geriatric Nursing Review Syllabus for advanced practice nurses published by the American Geriatric Society over this 2-day program.

Caregiver Boot Camp

This 2-day education program combines didactic and case-based discussions with experiential learning sessions.

Based on the John A. Hartford Foundation's Geriatric Patient Care Associate (GPCA) course curriculum (2nd ed.) the program will present components from each 10 essential modules. Case-based discussion based on the "How to Try This" video presentation series will augment didactic presentations.

The intent of this 2-day program is to address the evidenced-based program core while addressing the unique needs of lay and family caregivers. It will be offered at least three times in each year of the grant.

Care Manager Boot Camp

This 2-day intensive program will focus on the essential knowledge for care managers working with older adults within payment models such as Accountable Care Organizations.

The course will address strategies to provide health coaching to patients and families and provide care consistent with quality measures and the triple aim of better care, better health, and reduced costs. The program will focus on working in a patient-centered medical home environment or with other population health initiatives.

Geriatric care managers will learn how to maximize the health and function of older adults and strategies to promote wellness and prevent disability. Using a case-based approach, participants in the program will promote patient engagement and empowerment and provide effective care that reduces the cost of care while improving the quality.

The program will utilize or adapt existing curriculum for a number of insurers who have developed training programs and in collaboration with programs at academic organizations designed to address this educational need.

LNA Boot Camp

This three-day education program combines didactic and case-based discussions along with experiential learning sessions.

Utilizing and expanding on the John A. Hartford Foundation Institute for Geriatric Nursing's Geriatric Patient Care Associate (GPCA) course curriculum (2nd ed.). Case-based discussion based on the "How to Try This" video presentation series will augment didactic presentations.

The intent of this three-day program is to integrate the evidenced-based program core curriculum with patient-centered models of care such as Learn, Empower, Achieve, and Produce (LEAP) and Green House Workforce initiatives.

Concepts will come to life during experiential learning labs that include standardized patients, use of the Patient Safety Training Center, and the geriatric medical home learning laboratory as a member of an interdisciplinary team.

RN Boot Camp

This immersive continuing education program combines didactic and case-based discussions with experiential learning sessions including hands-on simulated or standardized patients combined with online learning experiences.

Based on the John A. Hartford Foundation Institute for Geriatric Nursing's Geriatric Resource Nurse Core Curriculum, this program will include the 14 content modules from Evidence-Based Geriatric Nursing Protocols for Best Practice.

The intent of this program is to integrate this evidenced-based core curriculum focused on geriatric syndromes with patient-centered models of care such as the Learn, Empower, Achieve, and Produce (LEAP) and Green House Workforce initiatives.

Concepts learned will come to life in experiential learning labs within the patient safety and training center and geriatric medical home learning laboratory as a member of an interdisciplinary team.
