New patients
Thank you for choosing our care team for your medical care. If you are a new patient, please ask your primary care provider (PCP) or other physician to forward a referral with all pertinent medical records. If you do not have a PCP or referring doctor, please call our appointment line to talk with our team. We would be happy to work with you to get you scheduled with our team. Please see our Referrals and Precertifications page for more information about referrals. A referral is not required to be seen in our practice, but it is important you check with your insurance provider if it is required to be covered under your plan.
Once we receive your information, we will contact you to schedule your appointment. We may recommend testing before your appointment, which we can help you coordinate. Some of our locations offer both telehealth and in-person appointments. The scheduling secretary will explain your options to you in detail depending on the location you choose.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.

To refer a patient for consultation, contact our Physician Connection Center. You also may contact us at one of our locations directly.
Hepatology Program referrals: If you are a provider who would like to refer your patient, please fill out the appropriate form below and fax it to the Hepatology Program. Please include all the information requested with the referral form.
