Some health insurance policies require that you have a referral from your family doctor (primary care provider, or PCP) to make an appointment. Please review your health insurance coverage before calling for an appointment.
For Female Pelvic Medicine and Reconstructive Surgery (Urogynecology) appointments, in most cases, you will be referred to our team by your PCP, but you can also call us directly for an appointment.
Returning patients
If you are a current patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record, and more.

For referring physicians
To refer a patient for consultation in the Department of OB/GYN, contact our Physician Connection Center. You also may contact us at one of our locations directly.
If you are a provider and would like to refer a patient to Family Planning, please fill out this Referral Form for Family Planning Services (PDF) and then fax it to us at 603-650-0904.
