New patients
We permit self-referrals, but patients are generally referred to the Palliative Care Program by a primary care provider or primary specialist.
If you feel that you or a family member would benefit from a palliative care appointment, please speak with your provider about a referral or contact our clinic for information.
For referring providers
To refer a patient to the the Palliative Care Program, contact our Physician Connection Center. You also may contact us directly.
