We help you and your family to understand your health condition and treatment options. We also assist your family in adjusting to the practical challenges and emotional stress of caring for someone they love who is seriously ill.

Advance directives are written instructions about your healthcare wishes and are stored in your medical record. It's helpful to have an advance directive when you have a health crisis that leaves you unable to speak for yourself. An advance directive is a gift to your family in a time of crisis and stress.

Care Partner Peer Support Group

Caring for someone with a serious illness can be complex. You may find deep meaning, and you may also feel a sense of isolation. Our in-person peer-support group can help connect you with others who understand what you're going through.
