New patients
If you are new to our practice and would like to schedule a consultation or get more information, please see your family doctor (primary care physician or PCP) first. Your family doctor will examine you, and then refer you to us for consultation if appropriate.
Note: You cannot set up an appointment without a referral from your family doctor.
Returning patients
If you are a returning patient, you may use myDH to communicate securely with your providers, request or change appointments, review your medical record and more.
