The Primary Care Advanced Practice Provider (APP) Fellowship program is a 12-month postgraduate training program for nurse practitioners and physician assistants. We invite interested candidates to learn more about our program.

Program overview
The Dartmouth Hitchcock Medical Center Primary Care APP Fellowship is a unique opportunity to engage in 12 months of immersive, hands-on clinical training and didactic education, designed to equip you with the knowledge, tools, and confidence to provide primary care.
Practice-based learning is the bedrock of the fellowship, which features precepted clinics with a small and consistent group of APP preceptors who are dedicated to your growth and development as a primary care provider. Your preceptors will provide at-the-elbow support during clinic sessions, instruct and proctor new skills, and reinforce the use of:
- Evidence-based medicine
- Patient-centered care
- Team-based management of complex patients
- Best primary care practices
Our practice at Dartmouth Hitchcock Clinics Heater Road – Family Medicine comprises a core group of physician assistants and nurse practitioners with a diverse range of skills and clinical interests. Many of our APPs also serve as researchers, faculty, and educators, bringing academic depth and innovation to the practice. Fellows will benefit from this diverse expertise while working in a collaborative, team-based environment that values mentorship, quality improvement, and comprehensive care across the lifespan. Additionally, you will have the opportunity to learn from specialists throughout Dartmouth Health and share didactic content alongside both General Internal Medicine and Family Medicine physician residents.
Program leadership
Kiersten Wulff, DNP, APRN, FNP-BC, Director and Faculty, APP Fellowship
Kiersten Wulff is a Family Nurse Practitioner at Dartmouth Health – Heater Road Family Medicine and serves as the Director of the APP Fellowship. She was pivotal in bringing the fellowship to fruition and remains deeply committed to its success. Kiersten has extensive experience caring for patients across the lifespan, with a particular passion for trauma-informed care and women’s reproductive health. In her role as director and faculty, she is dedicated to fostering growth, resilience, and excellence in the next generation of advanced practice providers.
Indie LeClair, MSN, APRN, FNP-C, PMH-C, Interim Director and Faculty, APP Fellowship
Indie LeClair is a Family Nurse Practitioner at Dartmouth Health – Heater Road Family Medicine, where she also serves as a clinical team leader. She is the Interim Director and faculty for the APP Fellowship, committed to advancing APP education, mentorship, and professional development. In addition to her clinical and fellowship leadership roles, Indie is an Adjunct Professor of Nursing at Colby-Sawyer College and a Simulation Clinic Preceptor for Geisel School of Medicine at Dartmouth. She has extensive experience caring for patients of all ages, with a strong focus on maternal and child health, as well as improving access to primary care.
Cory Howarth, MSN, APRN, FNP-C, APP Lead, Primary Care Heater Road
Cory Howarth is a Family Nurse Practitioner at Dartmouth Hitchcock Clinics Heater Road – Family Medicine, where he serves as the Primary Care APP Lead. He is the Medical Director for the Lebanon-based Mobile Integrated Health Paramedic program, a partnership between the City of Lebanon Fire Department and Dartmouth Health. Cory has extensive experience in the community and population health spheres, with a focus on bridging connections between community health programs and Primary Care services, to expand the provision of primary care beyond traditional office-based settings into local underserved communities.
Credential awarded
A Certificate of Completion will be issued by the Department of Family Medicine and signed by the Medical and Program Director(s).
Financial information
APP fellows will be paid a competitive stipend and are eligible for full and comprehensive benefits provided by Dartmouth Health. The fellowship provides supplies and educational materials, including lab coats, textbooks, handouts, medical library access, and internet access while in one of our facilities.
Over 150 hours of Category 1 CME are available for free throughout the year by participating in or attending lectures, conferences, grand rounds, and journal club.
